Jeffrey Forman MD

Jeffrey Forman, MD, FACR is board certified in therapeutic radiology by the American Board of Radiology. He earned his medical degree from New York University School of Medicine. He completed his medical internship, residency and fellowship in Radiation Oncology at Johns Hopkins Hospital in Baltimore Maryland.

Dr. Forman has held faculty appointments at Wayne State University, where he was chairman, The University of Michigan, Memorial Sloan-Kettering Cancer Center and Cornell University Medical College.

Prior to joining 21st Century Oncology as the medical director of the Michigan Region, Dr. Forman was the medical director of affiliate programs and satellites for the Karmanos Cancer Institute. Dr. Forman is significantly involved in his professional societies, which include ASTRO, ASCO, ACS, and RTOG and was the past chairman of radiation oncology and the OA Center for SWOG.

He has been awarded numerous honors and awards which include the James E. Constantine Award, Teacher of the Year at Memorial Sloan-Kettering Cancer Center, Best Doctors in America (1994, 1996, 1998, 2002, and 2004), Jewish National Foundation Tree of Life Award, Fellow of American College of Radiology, Israeli Cancer Association Award and the American Cancer Society Lane Adams Award.

Dr. Forman has been the principal investigator of 12 grants, and has authored more than 150 publications, 8 book chapters, and 116 abstracts. He has presented nationally, as well as internationally, regarding his work on cancer especially his work on prostate cancer. He has coauthored the 2007 NOA and AUA Guidelines for the Treatment of Prostate Cancer.

His special interested include prostate cancer, conformal irradiation, 3D treatment planning, IMRT, image guidance, hypofractionation and particle irradiation. His research has also included quality-of-life effects of treatment and he was responsible for the Lymphoma Radiotherapy service at Karmanos and for SWOG.
